Well...
Without going deep into the secrets of my heart, this is the only one I can think of.
I had a crush on this girl in my class. She was part of the school play. Along with me.
Since I had a crush on her I asked her a costume question.
Are you going to wear a mini skirt?
She said yes. I meant to think the following line but it slipped out.

One that comes to mind is a school play in which I stayed quiet for a very long time because I thought my line was supposed to be said by my partner >_> eventually she poked me and I realized it was my line

My dad pet another kid that he thought was me... Not cool, They were having a Yu-gi-oh tournament nearby too. Even though I wasn't directly involved I feel embarrassed knowing I was probably the cause of it.
Then the time I said hi to a lady that I thought was my mom... My dad and brother still won't let me forget that one.
This next one may not sound embarrassing but it was, really.
Then there was the time I tried to ask a girl out... I gave her a note and she said no. And my friends being the "experts" with women they tell me to write another note, I got pimpslapped through a note, AGAIN...And she STILL won't talk to me and its been almost 3 years.

...that my friend works at had a rather funny event a while back. Two people who worked there were well-known to be having a rather 'fruity' relationship, with maybe a bit of action in her office being on the cards at times.....
The lady was a manager and mass mailed round some general restructuring info, and how people should contact her for some things not others now.
Her lover replied with 'Excellent. Do I still come to you for biscuits and blowjobs?'. What a lovely message - so lovely in fact that his fingers felt inclined to click 'reply to all' and sent it around the entire office, which included over 100 support staff.

Once, though, I was riding down a hill on my bike. I forgot, however, that there were no brakes on it, since it was secondhand. I tried to skid to a halt but, to my dismay, I was coasting into a parking lot, in which I hit a parked car so as not to zoom through the middle and risk getting hit by someone else.
The car had an elderly couple in it, whose surprised looks were all it took to make me feel like my face was as red as a Christmas tree ornament.
I can't believe its me either!
