It's a DS cart sized flash card with a slot for a micro SD card to load roms, homebrew, etc..
I have one of those and a G6 lite for GBA games. So when I turn the power on my DS, I have at least 70+ games to play on the go.
I still buy alot of my games, especially the PW series, but it's nice to not lug around an extra case for the games. I almost lost my Tetris DS when I dropped my case and the cart almost fell into a sewage grate....
The majority of my game collection is at my parent's house and all I got in my apartment is my DS and Wii. I've been selling off quite alot of my collection so I can finally get a house.

As you can tell, I don't have a lot of games, because I don't really have the time to by new ones.
And I have more PS1 games as well, but they don't have their cases.
