God, finally someone else who has played or at least seen Indigo Prophecy! I actually mixed those two on my first run through the game, just to see if he would die. I was right.

I was on the last boss, I barely had enough potions or special what nots, to make it through.
If you've played, the cut scene before the boss was like 20 minutes,
I almost killed her with about .9999% of my health left then finally she gives me the finishing blow.
Never ever to play it again.
But that's about it. Or I've got a ton for Oblivion. Halo, or Smash brothers

Got to the last boss, and eventually, it came down to this;
Mario: 6HP
Flurrie: 20HP
Shadow Queen: 4HP
Mario had attacked, and Flurrie was my last partner. Flurrie used Lip Lock! It does 3HP. then the Queen killed Mario ;_;

I think it's the Lost Colont level aboard the Ark. Anyway, I go through a door, it closes. I remember this is the wrong way to go, so I turn around, and go back through the door.
Shadow falls to his death. WHAT IN HELL?

Here's some other ones:
- Tidus hits Lulu, Lulu slaps him across the face because she has counter attack, Tidus dies (FFX).
- Lara turning into gold on the Hand of Midas (Tomb Raider).
- Lara exploding when pressing the right buttons on the controller (Tomb Raider).
- Lara being crushed by a boulder (Tomb Raider).
- 10,000 NEEDLES!!!!!!!!(a few Final Fantasy games).
- Repeatedly hitting Wylaf after he says to WITHDRAW YOUR WEAPON, THE FIGHT IS OVER (Threads of Fate).
- Devour (FFVIII)
- A glitch I encountered trying to summon the Magus Sisters, that made Tidus and Wakka run away as they're supposed to, but made Yuna run around in circles FOREVER until I just sighed and pressed the reset button. (FFX).

1) When you are in the same teleporter as Gordon Freeman, if you attempt to jump into the same portal, you'll actually be causing a rift and you'll be teleported over nothing just to die.
2) When you get your hands on the teleporting gun, in certain levels, if you try to teleporting, sometimes you'll actually die. Ah well, that's expected from a prototype...
3) This one was hilarious to watch but if you defeat the big blob of purple insect-like monster in a certain way, it'll actually disintegration but the electricity is there so you can lure aliens into it and they'll actually die from it.
4) Obviously, electricity and water doesn't mix but I accidently used an electro-gun and zapped myself...that was rather hilarious since if you used the same gun at a lake filled with enemies, they don't react...damn...
