I was playing Tales of Symphonia a while back, and it was... I think Colette, Genis, Lloyd, and Kratos vs. a Mimic. I was Lloyd. First fight, all of us died. Second fight, same thing. Third one, same again.
Then I switched my character to Genis.
First fight: Killed it in 20 seconds.

We was going to lose and I was on of last two soldiers. Suddenly, someone throw granade on my feets and I "fly".
Then someone kills that last soldier of us and we get that DEFEAT text on screen. Now to the funny part...
When you lose or die, the camera jumps to some random place. Well...when this happened it showed my soldiers face. The whole screen just one rebel soldiers face...what is the most terrible part? It had somekind odd smile on his face...
I still see dreams of that...

Was playing Grand Theft Auto: Vice City, and I was sitting inside a car with very low health, listening to "I Ran" ba A Flock of Seagulls. Suddenly, a hooker pulls me out of the car, and kills me! I was like "..... errr........ ROFL!!!"

I was running after this guy, and I was about to knife him, when suddenly he pulls a golf club out of nowhere and whacks my me upside the head. So then I stab him, but he doesn't die, so I stab him again, and then this other random guy just comes out of nowhere and kills me.

As for me, I was screwing around with CORRUPT and Super Mario Bros. I got through the first level (the enemies had turned into Bullet Bills. Hilarious.) and I went through the pipe to 1-2 (with Koopas on the other side. Stuck.)
Well, I appeared, and there was only ceiling. No floor. No wall. Fell to my death. (This also happened with Game Genie fixes)
There's tons more from newer games, but I can't quite remember them all.
