Most male characters just have the obvious things attached when you genderswap them.
But sometimes when it's female-male (specially with younger charcters) it's a face change.

Yes, they do.
For example, Female Minuki has a face change, but Odokori's is largely untouched (other then the slightly larger eyes and lashes).
This has got me onto what would happen if Maya/PEarl channeled a male charcter...

I'm always debating over what name Neil would have as a female. Either Nelly (as in, "Whoa, Nelly!" and it contains "Ne" and that long E sound) or Annie (which contains N, E, and I, could be a reference to Annie Oakley, or could be used for an awkward Smooth Criminal reference). Possibly Nina.

It still looks too much like Mia, if you ask me. And it is a bit sloppy. Do you have some tips to make it better?
It's looking good but I do have a few suggestions...
1) Lose the Magatama, Thet will really help great rid of Mia impression and maybe replace with a band-aid or something.
2) Make the Scarf around the neck white/tannish like the undershirt.
3) Thin the Eyelash, They seem a little to thick to me.
