From left to right its Kyousaku Kawadzu, Shoutarou Hamigaki, and Mike Meekins. The first two are side characters in GS4. OT3 means "One True Threesome," which is a variation on OTP ("One True Pairing").
It means I think it'd be AWESOME if they were together.
