It's an incredibly badass story, and has been made into countless forms of media. The original novel, a Chinese TV show, video games, there's a manga and probably an anime.
I'm currently reading a children's version of it in Chinese complete with hilariously awesome pictures (all very 'cute' but still featuring people getting like sword'd and arrow'd and stuff).
It's an awesome story.

There's the rather infamous Ikkitousen, and then now there's Koutetsu Sangokushi (where the characters keep their genders, but are all turned into bishounen with plenty of shounen-ai vibes). And then there's a game out there named Koihime which is the usual "everyone is a pretty girl" (and Zhuge Liang looks ten years old or something), except for Diao Chan, who should probably not be seen.

If I compared correctly (and by compare, I mean read the bios of almost every character in Koei's ROTK VIII-X), it's a mix of both.
Some events within the historical battle have changed.
Example: Guan Yu
did NOT kill Hua Xiong 
at Si Shui Gate. Sun Jian's 
forces did.Some characters in the book are fictional (but we may never know).
Example: Zhou Cang
(apparently Guan Yu's right hand man)Example: Ma Yunlu
(Ma Chao's

)However, what can you expect from a dark era "filled with hopelessness" and whatever (-> trying to quote Koei's RoTKVIII) that is a mix of historical records and guess work. Take as much of history as you can and leave the gaps to oral folklore and the imagination, I guess...
